Pharmacy Team
The PCN pharmacy team is made-up of Pharmacists and Pharmacy technicians. We support the safe and cost effect use of medication for patients across the PCN. In the GP practices and the care homes our services include, medication reviews, providing information on medication to patients and staff, supporting the monitoring of drug treatment to promote the safe use of medication and helping with prescriptions and the supply of medication. We also carry out home visits to support patients with their medication. You can contact the team to discuss your medication, through your GP surgery.
